The Chronic Pain Care Coordinator assists in coordinating appointments for pain management as well as coordinating specialty appointments such as;

• Orthopedics
• Neurosurgery
• Physical or Occupational Therapies
• Pain Clinic
• Mental Health Counseling

The Chronic Pain Care Coordinator also provides the following services;

• Serves as point of contact between patient and Provider for any questions or communication regarding the plan of care for chronic pain management
• Provides information related to pain medications, side effects and disease processes
• Assists in arranging alternate non-pharmacological treatment modalities such as dry needling, auricular acupuncture, massage or chiropractic services
• Assists patients in managing pain, optimizing function and maximizing quality of life with improved physical and psychosocial function
